We are looking for Care Manager Interns to provide continuous care coordination, supporting chronically ill patients through Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) Programs. This role combines both administrative and clinical responsibilities in a dynamic, fast-paced environment. Hybrid position – Monday through Friday. Minimum commitment of three months is required. Please note that you must be available for in-person meetings as well as visits to the practices once or twice a year if needed. W2 Only, No C2C. Email to alisha@luzontech.com
