We are looking for a dedicated Lead Care Manager to provide continuous care coordination, supporting chronically ill patients through Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) Programs. This role combines both administrative and clinical responsibilities in a dynamic, fast-paced environment. This is a full-time, hybrid position working Monday through Friday. Please note that you must be available for in-person meetings as well as visits to the practices once or twice a year if needed.
